Chesapeake Health Advocates was created to provide guidance, expertise and liaison services to patients and their families when a healthcare crisis or life transition is occurring. The company is a source of information regarding patient safety issues. Through education and coaching, the staff members of Chesapeake Health Advocates teach patients and their families how to be confident when making healthcare decisions.

- performing an intake interview for every patient to ascertain patient and family needs.
- assisting in management of chronic health conditions/ chronic disease health coaching
- researching the patient’s illness or condition to garner information regarding best practices and newest treatment options.
- helping the patient more fully understand their condition(s).
- accompanying the patient to doctor visits, inpatient and outpatient hospitalizations or treatments, and diagnostic studies.
- working as a liaison with healthcare providers.
- acting as a knowledgeable resource for the patient and family as they navigate the healthcare system.
- advocating for the patient and/or family in untoward circumstances.
